Forms for the School Year


Below are the forms that will need the attention of all new and returning members of the Eagle Pride.  
Students will receive all forms in person during band camp, and forms will be due by the end of Drill Camp.

1.Medical Consent Form 26-27: This will need to be printed and notarized before returning to the blue box in the band room by the first band summer rehearsal or tryout date.

2.  General information form 26-27:  Follow the link to complete the form.

3.On/Off Campus Activity Form: For allowing your student to attend all activities on and off of campus (away games, etc.).

4.  I Can Help form:  Follow this link to find ways and sign up to volunteer with the Eagle Pride!

5. Eagle Pride Attendance/Participant Contract: Required for all students and parents to sign.

6.Personal Appearance and Grooming Policies for Performances: Our “Grooming Policy” for performances.

7.Food Allergy Information Form

8.Dispersion of Medication Form - For Band Trips

Absence Request Form - this form must be completed, signed, and submitted to the Director. (Reminder: Forms are to be submitted 4 weeks in advance for marching band, 8 weeks in advance for concert band/MPA.)

Weighted Band Honors Credit Requirements - this form is required for all Band V or VI students.
